CHANGE MY PERSONAL DETAILS ?

It is important to let us know of any change of name, address or telephone number in order to enable your records to be kept up to date. If you leave the area it will be necessary to register with another doctor. You do this by completing the relevant section of your medical card and taking the card to a surgery of your choice.
